From enquiry to booking

WhatsApp Business pricing from 1 October 2026: service replies now carry a cost

From 1 October 2026, according to a Meta business partner, service replies inside the 24-hour window are billed above a monthly free allowance. How to organise your clinic's replies and keep them fast.

If your facility replies to patients on WhatsApp through a system connected to a Meta-approved provider, something in your bill changed this month. The change concerns the messages your team sends in reply to patient enquiries.

How it worked before October

Since 1 July 2025, Meta has charged per message instead of per conversation, according to its official developer pricing page. Utility templates, such as an appointment confirmation, were free when sent inside the customer service window, meaning within 24 hours of the patient's last message.

Templates are message formats Meta approves in advance, used for reminders and confirmations.

What changed on 1 October 2026

According to YCloud, a Meta business solution provider, in a post dated 4 September 2026:

  • Ordinary replies inside the 24-hour window, the messages your team types or your reply system sends, are billed at the utility or authentication rate for the patient's country.
  • Each phone number gets 1,000 free service messages a month.
  • Utility templates sent inside the window are now billable too.
  • The 72-hour free entry window continues. It opens when a patient messages you from an ad that takes them straight to WhatsApp.

Rates vary by country and are updated from time to time, so we leave the figures to Meta's official rate card and your provider's invoice. Check the rate card before doing any sums.

What this means for a mid-sized clinic

The free allowance covers many small clinics. Medical groups that receive hundreds of enquiries a day on one number will usually pass it in the first or second week of the month.

An illustration: one number receives 60 enquiries a day, and the team sends an average of 5 messages per enquiry. That is around 9,000 service messages a month, of which 1,000 are free and the rest are billed at the patient's country rate. If the average drops to 3 messages per enquiry, the total falls to around 5,400, with the same patients and the same quality of reply.

Cost aside, speed and clarity of reply remain the main factors in turning an enquiry into a booking. What needs organising is the number of messages, while keeping replies fast.

Practical steps

  1. Ask your provider for a report of service messages per number for September 2026, so you know where you stand against the 1,000 allowance.
  2. Put the reply in one complete message. A team that sends "hello", then "one moment please", then the price, then the time, uses 4 messages for one answer.
  3. Prepare written replies to common questions that combine the answer, one clarifying question and two available times in a single message.
  4. Review automated reminders. If a patient receives a confirmation, two reminders and a post-visit message, ask which of them actually reduces no-shows, and keep that one.
  5. If you run ads that take patients to WhatsApp, aim to settle the booking inside the free 72-hour window.
  6. If you have several numbers for different branches, count the free allowance for each number separately.

How to measure the effect

Two numbers are enough, reviewed monthly:

  • Service messages per booking: total service messages in the month ÷ bookings that came through WhatsApp.
  • WhatsApp cost per booking: the monthly WhatsApp bill ÷ bookings that came through it.

Compare October with September. If cost per booking rises, start with step 2, which is usually the quickest saving. At the same time, keep an eye on average time to first reply, so you know the savings leave reply speed intact.

In practice

The change rewards a clear, complete first reply, which is what patients prefer anyway. Start with September's report from your provider, then adjust your prepared replies and reminders. If you would like to review the full messaging path at your facility, from enquiry to attendance, the Naqar team can go through it with you.
