Ranking in search and AI answers in 2026: what Google, Bing and the research say

Google says trust carries the most weight on health pages and that AI answers need no special optimisation. What that means for clinic content, and how to measure AI citations.

9 October 2026 · 4 min read

Google's current guidance is direct: trust is the most important part of how it judges content quality, and health pages receive even more weight on it. Google also says AI Overviews and AI Mode need no special optimisation beyond being indexed and eligible for a snippet. For a clinic, the work is named authorship, direct answers and real sources, measured through the reports Google and Bing now provide.

What Google says

Google's page on creating helpful, reliable, people-first content, updated on 5 October 2026, makes four points that apply to clinics:

  • Trust is the most important part of E-E-A-T (experience, expertise, authoritativeness and trust).
  • For health and other topics that affect people's wellbeing, Google's systems give "even more weight" to strong E-E-A-T.
  • Where readers would wonder how content was made, disclose the use of AI.
  • Mass-produced AI content created to manipulate rankings breaks Google's spam policies.

Its page on AI features and your website, updated on 10 December 2025, adds that there are no extra requirements for AI Overviews or AI Mode, and no AI-specific text files or new markup are needed. Traffic from these features is counted in Search Console under "Web".

What Bing shows

Bing now reports AI citations directly. Its AI Performance report, released in public preview on 10 February 2026, shows how often your pages are cited in Copilot and Bing's AI summaries, which pages are cited and the queries behind them. Bing notes that the data is a sample and does not measure rankings.

On 16 June 2026 Bing added Intents, Topics, Citation Share and Compare to the same report, in preview.

IndexNow lets a site notify participating engines (Bing, Naver, Seznam.cz, Yandex and Yep) when pages are added, updated or deleted.

What research adds

The paper "GEO: Generative Engine Optimization" by Aggarwal and colleagues, accepted at KDD 2024 (arXiv), tested ways of writing content for AI answer engines. Methods such as adding cited statistics and clear sources raised visibility in generated answers by up to 40% in their benchmark, and results varied by subject. This is a laboratory result on a test set. It shows which writing methods helped; the effect on any single site will differ.

Where llms.txt fits

llms.txt is a proposal by Jeremy Howard from September 2024 that describes itself as informal. Google's AI features page says such files are not needed for Google. Adding one is low-cost and optional. Treat it as a courtesy file, outside your ranking plan.

Our article on how clinics appear in AI answers covers the page-level basics behind all of this.

Practical steps

  1. On every health article, show the named medical reviewer, their credentials, the review date and the sources. This answers Google's questions about who made the content, how and why.
  2. Open each section with a short, direct answer, then give the detail. Use clear headings and cite real statistics where they exist.
  3. Keep structured data identical to what the page shows.
  4. Notify search engines with IndexNow when you publish or update a page.
  5. Disclose AI assistance where a reader would expect to know, and have a clinician check every medical statement.

The SEO & AI Visibility service builds these steps into each page.

How to check it is working

  • Share of health articles with a named reviewer, credentials, review date and sources. The target is 100%.
  • Search Console, "Web" search type: clicks and impressions for your service and article pages, monthly.
  • Bing AI Performance, monthly: number of citations, cited pages and grounding queries.
  • A fixed set of 10 patient questions put to AI tools each month, as described in our article on patients asking AI.
